Mark Rosenbloom, MD, MBA, FACEP, Adjunct Associate Professor of Medicine, Emergency Medicine, Feinberg School of Medicine at Northwestern University

Overall, 57,801 exposures to different types of vitamins were reported to the poison control centers across the US in 2003, accounting for 63 major adverse outcomes and 4 deaths. Of the total exposures, 45,352 exposures occurred in children younger than age 6 years.Mortality/Morbidity: Morbidity and mortality from pure vitamins are rare. One study of acute or chronic overdoses, with more than 50,000 exposures, reported 4 deaths and 63 major adverse outcomes.Race: No scientific data indicate that outcomes of vitamin overdose are dependent on race.Sex: No scientific data indicate that outcomes of vitamin overdose are dependent on sex.

Increase in Poisoning Deaths Caused by Non-Illicit Drugs — Utah, 1991–2003

Deaths caused by drug poisoning of unintentional and undetermined intent are an increasing problem in Utah and elsewhere in the United States (1). To characterize the trend in drug-poisoning deaths in Utah, CDC and the Utah Department of Health analyzed medical examiner (ME) data for 1991–1998 and 1999–2003. This report summarizes the results of that analysis, which determined that, during 1991–2003, the number of Utah residents dying from all drug poisoning increased nearly fivefold, from 79 deaths in 1991 (rate: 4.4 per 100,000 population) to 391 deaths in 2003 (rate: 16.6). This increase has been largely the result of the tripling of the rate (from 1.5 during 1991–1998 to 4.4 during 1999–2003) in poisoning deaths of unintentional or undetermined intent caused by non-illicit drugs (i.e., medications that can be legally prescribed) (Figure). Further study is needed to understand these trends and to develop strategies to prevent deaths of unintentional or undetermined intent from non-illicit drug poisoning.

April 12-18; Death by Medication

Published: April 19, 1998

More than 100,000 people die in American hospitals each year from adverse reactions to medication, researchers say, revealing a problem that is far more serious than the medical profession had realized.

The reactions are not due to mistakes by doctors or patients, but to the side effects of drugs. Severely ill people who take many different medications at the same time are most vulnerable, but experts say more careful monitoring by doctors may save lives. DENISE GRADY  - New York Times

Deaths from medication errors on increase

This is an article that I have recently seen on the Internet.

Report: Deaths from medication errors on increase February 28, 1998
Web posted at: 4:43 p.m. EST (2143 GMT)
(CNN) — Deaths caused by medication mistakes more than doubled between 1983 and 1993, according to findings published in the British medical journal The Lancet on Saturday.

The study by a research team at the University of California at San Diego found that the sharpest increase in deaths was among outpatients.

Deaths from accidental poisoning by drugs and other medicines climbed from 851 to nearly 2,100. Of those, outpatient deaths increased from under 200 to just under 1,500. By 1993, outpatients were 6.5 times more likely to die from medication mistakes than inpatients.

The report was based on death certificates, and it was unclear whether the deaths were caused by a medical professional’s error or patient error.
